FAP Forms
Below you will find the forms to make a submission to the Federal Appeals Panel.
- If you would like to file a case with the FAP, please complete Form 1A or Form 1B. 	
- Form 1A is used to appeal decisions made within the Federal Complaint Process.
 - Form 1B is for all other purposes.
 
 
- If you are responding to a case that has been accepted by the FAP, please complete Form 2.
 
- If you would like to file a procedural application, please complete Form 3. 	
- Below are some examples of reasons for procedural applications to be submitted: 		
- to request special arrangements for providing evidence,
 - to request permission to withold sensitive information from other parties,
 - to request an extenson of a deadline,
 - to request a change in directions already given.
